There are two ways of looking at accountability:
1. Keeping track of their actions and outcomes so that we can see who is not pulling their weight so that we can apply pressure and somehow force or motivate action. We can also learn who is weaker and further down in the heirachy.
2. Coming together to support each other, learn from failure, and create new strategies in order to collectively achieve the desired outcome.
The second is of course what we all want but when we fail to make it explicit and create a deliberate and repeated process for it the brain fears the first version.
This fear is demonstrated by vague communication, excuses, stress, illness, passing responsibility, hiding things, blame, complaining about circumstances eg lack of resources and often a feeling of isolation, loneliness or hopelessness and often a high staff turnover.
This is true whether holding yourself or others accountable.
